Request for Statutory Assessment

The following online form will enable you to request the Statutory Assessment of a child. It does not automatically lead to statutory assessment being initiated or to a statement of special educational needs being written.

It is important that you try to provide as much information as possible, in order for the Education Authority to process the request in a timely manner.
