Request for Statutory Assessment

As part of an important improvement to the Request for Statutory Assessment process, this portal is closing for requests from 5pm on Friday 12 December 2025.

This is to help prepare for the process moving to the EA Connect portal from 12th January 2026.

 

This will improve communication while maintaining a format that is familiar to families.

 

Should you want to submit a request for statutory assessment in the interim period, you can do so by completing a form, which will be available on the SEN website.
